Medical Social Worker

Job Summary:

Responsibilities: 

• Assess patient psychosocial and environmental needs 

• Assist with discharge planning and long-term care arrangements 

• Provide counseling and emotional support to patients and families 

• Connect patients with community resources and financial assistance programs 

• Complete required documentation per Medicare/home health standards 

• Participate in care coordination with interdisciplinary team 

Qualifications: 

• Master’s degree in Social Work (MSW) 

• Active Florida license (LMSW/LCSW preferred) 

• Home health or healthcare experience preferred 

• Strong knowledge of community resources 

• Excellent communication and organizational skills
