We are currently seeking a Social Worker Case Manager to join our team. In this role, you will provide high-quality, patient-centered case management services that support positive outcomes for patients, families, physicians, and staff.
As a Social Worker Case Manager, you will play a key role in coordinating care and discharge planning by applying strong clinical judgment, communication, and problem-solving skills. You will work collaboratively with internal teams and external agencies to ensure patients receive appropriate care and resources while maintaining a professional standard in all case management activities.
Key responsibilities include:
- Coordinating patient care and discharge planning to support optimal outcomes
- Collaborating with physicians, patients, families, and interdisciplinary teams
- Communicating effectively with insurance providers and community resources
- Applying critical thinking to assess, plan, implement, and evaluate care management services
- Demonstrating strong organizational, multitasking, and interpersonal skills
- Maintaining professionalism while working independently and as part of a team
QUALIFICATIONS:
Education:
- Social Worker with Bachelors degree required
Experience:
- Open to all new graduates
- Three to five years of hospital experience preferred
- Experience in case management or a related field preferred
- Experience in rehabilitation settings is a plus
Knowledge & Skills:
- Knowledge of care management, resource utilization, and utilization management required
- Strong critical thinking, communication, and organizational skills
- Ability to manage multiple priorities in a fast-paced environment
- Commitment to providing high-quality, patient-centered service
Licensure, registration or certification:
- Current licensure in the State of Kansas required
- Basic Life Support (BLS) certification required
- Specialty certification strongly encouraged within three years
