The Role:

Senior Director, Care Management has oversight of the managers and supervisors of Care Management located at each ministry and is accountable and responsible for collaboratively leading, coordinating and supporting care management operations in partnership with the Executive Director of Care Management and the Directors of Utilization, Appeals and Clinical Training.

What You'll Do:

  • For direct patient care roles: Performs and maintains currency of essential competencies as required by specific area of hire and populations served.
  • Adheres and complies with all policies and procedures.
  • Provides guidance and oversight to Care Management in addressing caregiver engagement, department operations, addressing length of stay and readmission opportunities for improvement, and developing strategies to ensure department and its caregivers are successful in meeting goals and outcomes.
  • Provides service area leadership and strategic planning to operationalize standard care management services within Prov-Swedish North Division and develop a coordinated approach to care management across the continuum.
  • Oversees service area development of goals, creation and implementation of action plans and program evaluation.
  • Sponsors and leads process improvement and change to achieve operational excellence. Seeks to identify systems, processes, practices and functions that are not working efficiently and/or effectively and actively leads correction. Supports multi-disciplinary work teams as necessary to correct and monitor results.
  • Participates in division and service area wide care management teams, acting as critical liaison and resource to internal and external stakeholders related to care management daily operations.
  • Is an active consultative resource and support for service area leadership, and immediately brings to attention of appropriate leadership areas of concern, along with requests for consultation and support from regional leadership.
  • Performs ongoing audits to monitor performance, ensure compliance to regulatory requirements, and escalates findings as necessary.
  • Works closely with identified service area physicians and chief nursing officers to coordinate efforts.
  • Ensures that all aspects of federal, state and local compliance are met.
  • Promotes and maintains positive relations with continuum of care providers.
  • Collaborates with Education in developing and updating of standard work and policies for Care Management.
  • Measures, analyzes and reports key metrics related to care management across the care continuum.
  • Facilitates problem solving with other departments as indicated.
  • Provides key stakeholders (managers, staff, physicians, community) education related to regulations/requirements or best practices.
  • Identifies opportunities to appropriately coordinate services between inpatient and outpatient settings.
  • Develop relationships with leadership across the continuum within the service area.
  • Responsible for Care Management standardization across the service area with the aim to decrease total cost of care.
  • Maintains confidentiality of all information related to patients, medical staff, employees, and as appropriate, other information.
  • Demonstrates service excellence and positive interpersonal relations in dealing with others, including patients/families/members, employees, managers, medical staff, volunteers and community members, so that productivity and positive relations are maximized.
  • Maintains knowledge of work-appropriate aspects of environment of care programs; complies with policies and reports unsafe conditions.
  • Responsible for development and oversight of consistent service area and system reporting for care management related outcomes. This includes strong collaboration for management, engagement and ongoing development of technology capabilities (such as EPIC) to support care management functions, productivity and other work requirements.
  • Responsible for oversight of complex case management issues, outliers, ethics issues.

What You’ll Bring:

Education & Credentials

  • Bachelor's Degree in Nursing
  • Master's Degree in Nursing, Business Administration, Health Care Administration, or related field
  • Master's Degree (Graduate of an accredited/approved school of nursing) (Preferred)
  • Coursework in case management, managed care, and/or utilization management and experience in varied cross-continuum care settings (acute, outpatient, other). (preferred)
  • Required upon hire: Alaska Registered Nurse License (Vendor Managed)
  • Required upon hire: National Provider BLS - American Heart Association (Vendor Managed)
  • Required within 1 year of hire: Accredited Case Manager (ACM) - American Case Management Association (Facility Managed) or required within 1 year of hire: Certified Case Manager (CCM) - Commission for Case Manager Certification (Facility Managed)

Experience & Expertise

  • 6 years of clinical/operational leadership experience that provided the opportunity to manage a variety of patient care services in a complex healthcare environment.
  • 5 years Director/Manager level experience in Case Management.
  • Experience in a variety of healthcare settings, from acute care inpatient facilities to outpatient clinics. (preferred)

Professional & Leadership Capabilities

  • Ability to interpret, analyze, and utilize data to facilitate service area-wide improvement in operating efficiencies with a focus on care delivery.
  • Knowledge of state and federal regulations, with extensive knowledge of Medicare/Medicaid.
  • Knowledge of clinical management of complex health care needs and diagnoses.
  • Ability to work cooperatively with others to use appropriate strengths, knowledge and cooperation to improve performance.

The full pay range is listed in accordance with applicable law. Final compensation will be determined based on qualifications, experience, organizational compensation alignment, and the approved hiring department budget for the position. This position may also be eligible for incentive compensation and benefits.
