Job Summary\n

Cuready Healthcare Staffing is seeking a qualified RN, Outpatient Clinic to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities\n

\n
• Provide chronic disease case management for adult day health program participants.
\n
• Monitor medical conditions in conjunction with the interdisciplinary team and primary care providers.
\n
• Educate and support caregivers to prevent nursing home placements and reduce hospitalizations.
\n
• Utilize Turbo TAR EMR system specific to California Adult Day Health Care industry.
\n
• Adjust and adhere to established patient condition parameters provided by primary care providers.
\n
• Manage a daily patient ratio of 1:40.
\n

Qualifications\n

\n
• Current Registered Nurse (RN) license in California.
\n
• Minimum of 1 year RN experience; clinic, primary care, or home health experience preferred.
\n
• Basic Life Support (BLS) certification required.
\n
• Experienced in using EMR systems; prior experience with Turbo TAR is an advantage.
\n
• Strong communication skills and ability to work collaboratively with interdisciplinary teams.
\n
• Must have reliable transportation and be willing to travel locally as needed.
\n
