The Health Center at Bloomingdale, a five-star skilled nursing facility, offers short and long-term nursing and rehabilitation care in a secure and caring environment. Responsibilities:

• Assist with resident assessment and implementation of care needs and will participate in resident and family care conference
• Must be able to effectively communicate with residents, families, staff, physicians, outside providers and the public
• Ensures that changes in resident condition are evaluated, monitored, documented and effective interventions are implemented
• Provide support and assistance to medication management systems
• Ensure that medications are dispensed in a safe and effective manner, keep medication cart orderly, and all medications appropriately packaged and stored
• Monitor resident health concerns including weight and nutrition, skin integrity, mobility, fall management and pain management

Requirements:

• Current licensure as a Practical Nurse ( LPN ) or Registered Nurse (RN) in the State of New Jersey
