• Plan, organize, and provide personalized home care services
• Conduct thorough patient assessments and develop customized care plans

• Maintain accurate clinical notes and ensure compliance with care standards

• Licensure: Valid RN license from the Arizona State Board of Nursing
• Experience: Minimum one year in home health or hospice preferred
• Transportation: Valid Arizona driver’s license and reliable vehicle
• Certifications: Current CPR certification & negative TB test (assistance available)
• Background Clearance: Satisfactory fingerprint clearance per federal/state regulations
• Bilingual Preferred: Conversational Spanish skills highly desired

• Associate’s Degree in Nursing or higher
