Nurse Care Manager (Part Time)

Martha's Hands is expanding our team of Nurse Care Managers for Private Pay Home Care Clients!

Martha's Hands Nurse Care Managers:

Utilize your Nursing Skills as part of a 12-person Care Supervisory Team to:

• Guide a client and their family through their care needs.
• Support/Train our Caregivers in providing direct care to our clients in their homes.
• Partner with home health, hospice and other care professionals to manage their care.

Overview:

Private Pay Clients Only - No Medicare or Insurance Documentation!

Our Nurse Care Managers are compassionate, health care professionals who:

• Conduct Assessments with clients and their families in order to develop a customized Service Plan.
• Coordinate care with other health care professionals including: Home Health, Hospice, Primary Care, etc.
• Orientate and train our Caregivers to provide excellent care.
• Provide clinical oversight of our clients and support to our caregivers.
• Provide direct care to our clients - perform Skilled Nursing tasks and ADL assistance as needed, including Medication Management, Wound Care, Diabetic Care, Tube Feedings, etc.
• Assist families with more detailed Care Management Services as needed.
• Balance both an office and mobile work environment, utilizing technology to enhance care for our clients and equip our Nurse Care Managers to be more efficient and effective.
• Supervise care team to ensure quality care expectations are being met while inspiring and encouraging our Caregivers.

• 3+ years nursing experience. Ideal experience in Private Duty, Home Health, Hospice, Geriatric Nursing or Med Surg.
• Team Approach to care by utilizing strong professional, interpersonal communication skills.
• Strong clinical assessment skills to develop a customized service plan that directs the care for our clients.
• Educational and training abilities to enhance Caregiver's success in caring for our clients.
