Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring an Intake Registered Nurse (RN) – Float for Element Care PACE in Lowell, Massachusetts.

POSITION OVERVIEW

• Complete comprehensive in-home nursing assessments for prospective PACE participants.
• Evaluate medical, functional, psychosocial, and environmental needs to determine program eligibility.
• Collaborate with the interdisciplinary team to develop individualized plans of care.
• Serve as a float RN supporting multiple PACE centers throughout the service area.
• Coordinate with physicians, therapists, social workers, and community partners to ensure seamless participant transitions.
• Maintain accurate documentation utilizing the electronic medical record.

COMPENSATION & BENEFITS

• Competitive salary from $80,000-$103,000 based on experience.

Requirements:

MINIMUM REQUIREMENTS

• Active Registered Nurse (RN) license in Massachusetts.
• Minimum one year of geriatric nursing or adult mental health nursing experience.
• Ability to complete comprehensive in-home nursing assessments.
• Strong assessment, communication, and documentation skills.
• Comfortable working independently while collaborating within an interdisciplinary team.
• Electronic medical record experience.
• Reliable transportation for travel between participant homes and multiple PACE sites.
