Position Highlights:

• Monday–Friday schedule, typically 8:00 a.m. to 5:00 p.m.
• Flexible scheduling options
• Interdisciplinary team meetings held every other Wednesday

Responsibilities Include:

• Assessing patients' physical, emotional, and psychosocial needs
• Educating patients, families, and caregivers on disease progression and care plans
• Managing pain and symptom control to maximize comfort and quality of life
• Coordinating care and serving as the primary case manager for assigned patients
• Collaborating with an interdisciplinary team that includes physicians, social workers, chaplains, CNAs, therapists, and volunteers

As a Hospice RN Case Manager, you will work under the direction of the Medical Director and Clinical Director while building meaningful relationships with patients and families during a critical stage of life.
