Job Title: RN Case Manager – Lead with Compassion in Hospice Care
Pay: $90,000 – $130,000 DOE
Benefits: Full Benefits Package
Location: Greater Twin Cities Metro Area, 21-County Service Area (territories assigned based on coverage needs)

What You’ll Do
• Develop and manage individualized hospice care plans for your patients.
• Monitor and manage pain, symptoms, and overall comfort.
• Serve as the primary RN and trusted guide for patients and families.
• Navigate complex situations with calm, confident clinical judgment.
• Coordinate closely with physicians, social workers, chaplains, and aides.
• Provide care in homes, assisted living, and skilled nursing environments.
• Maintain accurate, timely documentation to support quality outcomes.



What We’re Looking For
• Active Minnesota Registered Nurse license.
• Experience in hospice, oncology, or chronic illness management.
• Strong communication, organization, and problem-solving skills.
• Ability to work independently while staying connected to the team.
• Deep commitment to dignity, autonomy, and patient-centered care.
