DESCRIPTION OF WORK

This position focuses on providing steady support to consumers receiving Blended Case Management services. You will stay closely connected with families and service providers to help consumers meet treatment goals and access needed resources. As a County Caseworker 2, you will perform the following duties:

  • Treatment Monitoring: Evaluate treatment progress through regular contact with consumers and families
  • Service Coordination: Collaborate with service providers to document care and ensure needs are met
  • Crisis Response: Provide crisis intervention and emergency support when required
  • Advocacy Support: Assist consumers in accessing economic, health, vocational, and other resources
  • Caseload Leadership: Guide professional staff or student interns when serving as a lead worker
  • Information Gathering: Collect background information to identify social, emotional, economic, or physical needs

Work Schedule and Additional Information:
  • Full-time employment.
  • Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 1-hour lunch.
  • May have overnight and out of county travel for meetings or training.
  • May work irregular hours often and evening and weekends may be required.
  • Provides 24 hour/days per week on-call coverage on a rotating basis once trained.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS
Minimum Experience and Training Requirements:

  • Six months of experience as a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • A bachelor's degree with a social welfare major; or
  • A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; or
  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
  • Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

Other Requirements:
  • PA residency requirement is currently waived for this title.
  • You must be able to perform essential job functions.

Legal Requirements:
  • A conditional offer of employment will require a drug screening.
  • You must pass a background investigation.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
