Job Summary\n

Cuready Healthcare Staffing is seeking a qualified Registered Nurse, Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities\n

• Perform comprehensive patient assessments to identify needs for care coordination and discharge planning.
\n
• Organize and facilitate family meetings to ensure cohesive care planning.
\n
• Develop and implement effective treatment plans in collaboration with healthcare teams.
\n
• Coordinate care with HMOs, IPAs, and various healthcare providers.
\n
• Arrange durable medical equipment and transportation services independently.
\n
• Maintain accurate documentation using electronic health records, ensuring compliance with CMS and The Joint Commission guidelines.
\n

Qualifications\n

\n
• Current RN license in the state of California; eligible upon start date if pending.
\n
• Minimum of two years of acute care case management or discharge planning experience required.
\n
• Proficiency in the use of electronic health records, preferably Epic.
\n
• Previous travel nursing experience is highly preferred.
\n
• Demonstrated understanding of needs assessment, prior authorizations, and care coordination.
\n
• Ability to manage a caseload of up to 25 patients effectively.
\n
