Position Summary:
\n We are seeking an experienced RN – Case Manager with 2+ years of outpatient clinic experience to join our team. The ideal candidate will provide comprehensive case management services for frail elderly individuals, younger adults with disabilities, and patients with Alzheimer's disease and related dementias. Responsibilities include coordinating chronic disease management, monitoring patients' medical conditions in collaboration with the Primary Care Provider (PCP) and the Interdisciplinary Team (IDT), and providing caregiver education, support, and community-based care coordination to promote positive health outcomes and improve quality of life.

Roles & Responsibilities:\n

• Conduct comprehensive patient assessments and develop individualized care plans for frail elderly, disabled adults, and patients with Alzheimer's disease and related dementias.
\n
• Coordinate chronic disease management by collaborating with Primary Care Providers (PCPs), the Interdisciplinary Team (IDT), specialists, and community resources.
\n
• Monitor patients' medical conditions, medications, and care needs to ensure timely interventions and continuity of care.
\n
• Educate and support patients, families, and caregivers on disease management, treatment plans, available resources, and preventive care.
\n
• Maintain accurate clinical documentation, coordinate follow-up appointments, and ensure compliance with organizational policies and regulatory standards.
\n

Skills:\n

\n
• Strong case management and care coordination skills in an outpatient clinical setting.
\n
• Experience managing chronic diseases and coordinating care with PCPs, IDTs, specialists, and community resources.
\n
• Excellent patient assessment, care planning, and clinical documentation abilities.
\n
• Strong communication, caregiver education, and interpersonal skills with a patient-centered approach.
\n
• Ability to prioritize multiple cases, think critically, and ensure continuity of care while maintaining regulatory compliance.
\n
