Location: Apple Valley, California

\n

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30). Weekend rotation required (every other weekend). Patient ratios: 1:25 on weekdays, 1:45 on weekends. Caseload up to 25 patients daily.

• 13-week travel assignment at an acute care facility
\n
• Work with diverse patient populations including neonates, adolescents, adults, and geriatrics
\n
• Collaborate with interdisciplinary teams including physical therapy and social services
\n
• Epic charting system utilized for computerized documentation
\n
• Business attire or scrubs accepted
\n
• 40 hours of orientation provided
\n

Requirements:\n

\n
• Active California RN license (pending license accepted, but must be active by start date)
\n
• Minimum 2 years of acute care case management or discharge planning experience in a hospital setting
\n
• Previous travel nursing experience required; first-time travelers cannot be considered
\n
• Epic charting system experience required
\n
• Experience with patient assessment, care coordination, discharge planning, and utilization review
\n
• Competency in arranging DME, SNF placement, and ambulance transport independently
\n
• Knowledge of HMOs, IPAs, advance directives, POLST, and community resources
\n
• Ability to organize and participate in family meetings and treatment plan development
\n
• Hospital-based experience required; behavioral health experience alone does not qualify
\n
• Must commit to working 3 of 5 major holidays during assignment period
\n
• Two professional references covering one year within the last three years
\n
