We are currently seeking a compassionate and motivated Physical Therapist (PT) to join our rehabilitation team and help improve the quality of life for our residents through exceptional therapy services.

Your Role

• Provide physical therapy services to residents in a skilled nursing and long-term care setting

• Complete accurate and timely documentation

• Develop and implement individualized therapy treatment plans

• Collaborate with interdisciplinary healthcare teams to support resident progress

• Follow all facility policies and procedures

• Ensure compliance with all state and federal regulations and therapy standards

• Promote resident independence, mobility, and overall quality of life

Who We’re Looking For

• Current Physical Therapist (PT) license in Colorado or a compact state

• Compassionate and resident-focused professional

• Strong communication and teamwork skills

• Organized, dependable, and motivated

• Passionate about delivering high-quality rehabilitation services
