Occupational Therapist (OT) – Per Diem

Location: Pine Acres Center for Nursing & Rehabilitation

About the Role

Pine Acres Center for Nursing & Rehabilitation is seeking a Per Diem Occupational Therapist to support residents recovering from illness, injury, or functional decline. This role focuses on helping individuals regain independence in daily activities through structured therapeutic interventions in a long-term care setting.

You will work closely with an interdisciplinary clinical team to support rehabilitation goals tailored to each resident’s needs.

What You’ll Be Doing

  • Evaluate residents’ functional abilities related to daily living tasks.

  • Develop and carry out individualized therapy interventions focused on improving independence and safety.

  • Provide hands-on treatment to improve mobility, strength, coordination, and self-care skills.

  • Observe and adjust therapy approaches based on resident progress and response.

  • Document treatment sessions, outcomes, and functional changes in accordance with facility standards.

  • Coordinate care plans with nursing staff, physicians, and other therapy disciplines.

  • Educate residents and caregivers on adaptive strategies and home exercise programs.

  • Participate in care planning discussions and interdisciplinary meetings.

  • Assist with coverage across nearby facilities when needed based on census or staffing needs.

What We’re Looking For

  • Degree in Occupational Therapy from an accredited program.

  • Active OT license in the state of North Carolina.

  • Prior experience in skilled nursing, rehabilitation, or similar healthcare setting preferred.

  • Ability to safely assist with resident mobility and transfers (up to 50 lbs as needed).

  • Strong communication, clinical judgment, and organizational skills.

  • Ability to work independently in a flexible per diem role.
