Physical Therapist

A leading skilled nursing facility in Long Island is seeking a motivated and compassionate Physical Therapist to join our rehabilitation team!

Position Overview:
We’re looking for a PT who is passionate about helping others regain strength and confidence. In this role, you’ll collaborate with a team of healthcare professionals and play an important part in supporting residents on their journey toward improved function and quality of life.

Key Responsibilities:

• Conduct thorough evaluations, screenings, and individualized treatment plans.

• Deliver high-quality rehabilitative therapy focused on helping residents improve daily functioning and mobility.

• Document treatments, progress notes, and clinical tasks promptly and accurately.

• Educate residents, families, and staff on therapy techniques, mobility strategies, and care recommendations.

• Provide guidance and oversight to Physical Therapy Assistants as needed.

Requirements:

• Graduate of an accredited Physical Therapy program.

• Active New York State Physical Therapist license.
