Whether you're helping a resident walk independently again, return home safely, or achieve a personal milestone they never thought possible, you'll play a direct role in changing lives every day.

What You'll Do

• Evaluate residents and develop individualized treatment plans

• Provide evidence-based physical therapy interventions

• Improve strength, balance, mobility, endurance, and functional independence

• Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and families

• Assist with discharge planning and safe transitions home

• Educate residents, families, and caregivers on mobility and safety

• Maintain accurate and timely documentation

• Participate in interdisciplinary care planning meetings

• Help create a positive, motivating therapy environment where residents look forward to treatment

What We're Looking For

• Current Physical Therapist license in the State of Colorado

• Graduate of an accredited Physical Therapy program

• Strong clinical reasoning and assessment skills

• Excellent communication and teamwork abilities

• Passion for improving the lives of older adults

• Skilled Nursing Facility experience preferred, but not required

• New graduates encouraged to apply
