Responsibilities

• Provide skilled physical therapy services under the direction and supervision of a licensed Physical Therapist.

• Carry out individualized treatment plans designed to improve patients' mobility, strength, balance, function, and overall quality of life.

• Assist patients with therapeutic exercises, gait training, transfer training, balance activities, and use of assistive devices.

• Monitor patient progress and communicate changes to the supervising Physical Therapist.

• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques.

• Complete accurate, timely clinical documentation.

• Participate in case conferences, team meetings, and quality improvement initiatives.

• Collaborate with physicians, nurses, therapists, and other healthcare professionals to provide coordinated patient care.

• Promote safe, compassionate, high-quality care that helps patients remain independent in their homes.

Qualifications

• Graduate of an accredited Physical Therapist Assistant (PTA) program.

• Current Physical Therapist Assistant license in the state of practice.

• Current CPR certification.

• Minimum of two years of PTA experience preferred.

• Home Health experience is a plus but not required.

• Valid driver's license, reliable transportation, and auto insurance.
