• Graduate of an accredited Physical Therapy program
• Current state license as a Physical Therapist (PT)
• Strong clinical, communication, and documentation skills
• Experience in a skilled nursing or rehab setting preferred (new grads welcome!)
• Passion for providing high-quality patient care

• Evaluate residents and develop individualized treatment plans
• Provide therapeutic interventions to improve mobility, strength, and function
• Collaborate with interdisciplinary team members (nursing, OT, SLP, physicians)
• Monitor and document patient progress accurately and timely
• Educate residents and families on treatment plans and home exercise programs
• Ensure compliance with all regulatory and clinical standards
