About the Role:

We are seeking a compassionate and skilled Physical Therapist (PT) to join our team. As a Physical Therapist (PT) at Spring Hill Rehabilitation & Nursing Center, you will be responsible for designing and implementing individualized physical therapy plans to help our patients achieve their optimal level of function and independence. You will work closely with our multidisciplinary team to provide high-quality patient care and contribute to achieving outstanding patient outcomes.

Responsibilities:

• Conduct thorough initial and ongoing patient assessments to identify physical therapy needs and goals

• Develop and implement individualized physical therapy plans in collaboration with our patients, families, and healthcare team

• Provide physical therapy interventions to patients, including exercises, education, and manual therapy techniques

• Monitor and document patient progress, adjusting physical therapy plans as necessary

• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination

• Participate in patient care conferences and treatment team meetings to discuss patient progress and care plans

• Maintain accurate and timely patient records and reports

• Collaborate with other therapy disciplines (occupational therapy and speech therapy) to ensure comprehensive patient care
