Position Overview
As a Physical Therapist (PT), you’ll play a key role in evaluating residents, developing care plans, and delivering physical therapy interventions that improve mobility, relieve pain, and restore function. This position offers a rewarding opportunity to enhance the lives of residents through evidence-based, individualized therapy.

Key Responsibilities

  • Conduct assessments to determine mobility deficits, pain levels, and rehabilitation goals.

  • Design and implement individualized treatment plans to address each resident’s physical challenges.

  • Deliver skilled physical therapy services in accordance with physician orders.

  • Collaborate with interdisciplinary team members to support optimal patient care.

  • Educate patients, families, and caregivers on home exercise programs and safe mobility practices.

  • Maintain accurate, timely, and thorough documentation per facility and regulatory standards.

  • Participate in care planning meetings and provide recommendations for continued care.

  • Ensure compliance with all federal, state, and local regulations.

Qualifications

  • Active Texas Physical Therapist license

  • In good standing with all licensing and regulatory bodies

  • Experience in Skilled Nursing Facilities (SNF) or long-term care preferred

  • Familiarity with Net Health documentation system is a plus

  • Excellent communication and clinical reasoning skills
