We are seeking a Physical Therapist to join our team! You will plan physical rehabilitative programs to improve patient quality of life. 

Responsibilities:

• Develop and implement physical therapy programs for patients with strokes and neurological disorders that focus on rehabilitation for outpatient and home health treatment program.
• Work directly with patients in an outpatient, ALF and in home settings.
• Evaluate effects of therapy treatment and communicate patients' progress with their physicians and other healthcare providers.
• Record and document patient care services
• Collaborate with other team of therapists, nurses and physicians to achieve well-rounded coordinated care

Qualifications:

• Previous experience in physical therapy, rehabilitation in different settings, outpatients and home.
• Ability to handle physical workload
• Ability to build rapport with patients to achieve better results
• Excellent written and verbal communication skills
• Strong leadership qualities
• Licensed in the State of Florida
