Schedule: PRN (As Needed)

Pay Rate: Up to $65/hr (5-10 hrs C-PRN Weekly)

Oak Village Healthcare is seeking a licensed Physical Therapist (PT) to provide PRN therapeutic care in Oaktown, IN. This flexible role is perfect for therapists who want meaningful patient interaction while maintaining schedule flexibility.

• Perform evaluations and develop individualized physical therapy treatment plans

• Deliver evidence-based, patient-centered interventions

• Document care accurately and update goals as needed

• Collaborate with the interdisciplinary care team to support resident outcomes

• Maintain compliance, quality, documentation, and ethical standards

• Licensed Physical Therapist (PT) in the state of Indiana

• Current BLS certification

• Strong clinical reasoning and communication skills
