What You'll DoConduct comprehensive evaluations and create personalized treatment plans tailored to each resident's goals
Deliver evidence-based therapeutic interventions to improve mobility, strength, balance, and overall function
Monitor progress, celebrate achievements, and adjust treatment plans to maximize results
Collaborate closely with nursing staff, physicians, and interdisciplinary team members to provide holistic, high-quality care
Educate residents and families on treatment strategies, recovery expectations, and wellness goals
Maintain timely and accurate clinical documentation in accordance with regulatory standards
Graduate of an accredited Physical Therapy program
Current Physical Therapist (PT) license in good standing
Experience in skilled nursing, rehabilitation, or long-term care preferred
Excellent communication, interpersonal, and problem-solving skills
