Physical Therapist

Job Summary:

We are seeking a skilled and compassionate Physical Therapist (PT) to join our rehabilitation team within a skilled nursing facility. The Physical Therapist will be responsible for evaluating residents, developing individualized treatment plans, and providing therapeutic interventions designed to improve mobility, restore function, and enhance overall quality of life. The ideal candidate demonstrates strong clinical judgment, effective communication skills, and a commitment to high-quality, resident-centered care.

Responsibilities:

· Perform comprehensive physical therapy evaluations and develop individualized treatment plans

· Provide therapeutic interventions including therapeutic exercise, gait training, balance training, and functional mobility activities

· Assess resident progress and modify treatment plans as appropriate

· Supervise and direct Physical Therapy Assistants in the implementation of treatment programs

· Maintain accurate and timely documentation in accordance with facility policies and regulatory standards

· Educate residents, families, and caregivers on mobility, safety, and home exercise programs

· Collaborate with interdisciplinary team members to support comprehensive resident care

· Ensure compliance with all professional, state, and federal regulations

· Maintain a safe, clean, and organized therapy environment

· Ability to manage multiple patients and prioritize tasks effectively

Qualifications:

· Doctorate or Master’s degree from an accredited Physical Therapy program

· Current state licensure or eligibility as a Physical Therapist

· CPR certification required

· Experience in skilled nursing, long-term care, or geriatric rehabilitation preferred

· Strong clinical, communication, and organizational skills

· Recent graduates encouraged to apply
