Physical Therapist

About the Role: 

1. ADDAX HOME HEALTH LLC is looking for PRN passionate and skilled Physical Therapists/Physical Therapists Assistants to join our growing team in Fort Myers, FL. This is an exciting opportunity to make a meaningful impact by delivering high-quality, patient-centered care directly in the comfort of patients' homes. Our office is centrally located on Colonial Blvd. Our EMR System is Wellsky Kinnser.

 
Responsibilities: 

• Conduct comprehensive physical therapy evaluations and assessments in patients' homes
• Develop and implement individualized treatment plans to restore function and mobility
• Provide therapeutic exercises, manual therapy, and rehabilitative interventions
• Monitor and document patient progress, adjusting care plans as needed
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care
• Educate patients and caregivers on home exercise programs and injury prevention
• Maintain accurate and timely clinical documentation in compliance with regulatory standards

Requirements:
 

• Valid Florida Physical Therapist (PT) license in good standing
• Graduate of an accredited Physical Therapy program (DPT or MPT)
• Home health experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills with patients and care teams
• Reliable transportation and valid driver's license for home visits in the Fort Myers, Cape Coral, Lehigh Acres, Alva, Labelle, Punta Gorda, Estero, Bonita Beach area
• CPR/BLS certification required
