Physical Therapist

WELLBOUND Certified Home Health Agency is actively seeking skilled and compassionate Physical Therapists to provide services to pediatric and adult clients with special needs in Assisted Living Facilities across the five boroughs of New York City and Long Island. This role offers the opportunity to deliver meaningful, patient-centered care while supporting mobility, safety, and overall quality of life in a structured residential setting.

Location: All 5 boroughs and Long Island

Compensation: $100.00 per visit

Job Type: Fee-for-Service (FFS)

Responsibilities

• Design and implement individualized treatment plans addressing fine and gross motor skills, visual-motor integration, mobility, balance, strength, and conditioning.

• Support fall prevention initiatives and health and wellness programs for residents.

• Provide effective and timely communication with clients, caregivers, and interdisciplinary team members.

• Educate and train caregivers (including Home Health Aides) to ensure consistency and skill carryover.

• Complete clinical documentation accurately, professionally, and within required timeframes.

• Travel to multiple Assisted Living Facilities within the assigned service area.

Requirements

• Valid New York State Physical Therapist (PT) License (required).

• Strong knowledge of anatomy, physiology, and geriatrics to effectively treat diverse populations.

• Experience in clinics, skilled nursing facilities, assisted living, or home health settings preferred.

• Excellent communication and interpersonal skills to collaborate with patients, families, and healthcare teams.

• Ability to build rapport with patients while demonstrating empathy, patience, and professionalism.

• Strong organizational skills to manage multiple cases while maintaining high-quality care.
