Mekks Home Health Care LLC is seeking a compassionate and skilled Physical Therapist (PT) to join our growing home health care team. The ideal candidate will provide high-quality, patient-centered care to individuals in their homes, helping them regain mobility, improve physical function, and enhance their overall quality of life.  

Key Responsibilities 

•  Conduct comprehensive physical therapy evaluations in patients’ homes 
•  Develop individualized treatment plans based on patient needs and goals 
•  Implement therapeutic exercises, manual therapy, and mobility training 
•  Monitor patient progress and adjust treatment plans as needed 
•  Educate patients and family members on exercises, safety, and injury prevention 
•  Maintain accurate and timely clinical documentation in compliance with state and federal regulations 
•  Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care 
•  Ensure compliance with all home health care standards and company policies 

 
Qualifications & Requirements
 

•  Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program 
•  Active Massachusetts Physical Therapist License
•  Minimum of 1 year experience (home health experience preferred but not required) 
•  Strong clinical assessment and patient care skills 
•  Excellent communication and interpersonal skills 
•  Ability to work independently and manage time effectively 
•  Reliable transportation and valid driver’s license 

 
Preferred Skills
 

•  Experience with geriatric patients and chronic conditions 
•  Familiarity with electronic medical records (EMR) systems 
•  Strong organizational and documentation skills
