Job Title: Physical Therapist (PT)Position Summary

The Physical Therapist (PT) is responsible for evaluating, planning, and implementing individualized treatment programs for residents in a skilled nursing facility. The PT focuses on restoring function, improving mobility, relieving pain, and preventing or limiting permanent physical disabilities. This role ensures high-quality care delivery while maintaining compliance with regulatory and reimbursement guidelines.

Key Responsibilities
  • Perform comprehensive evaluations of residents’ physical conditions, functional limitations, and rehabilitation needs

  • Develop and implement individualized treatment plans based on clinical findings and physician orders

  • Provide therapeutic interventions including exercise, manual therapy, gait training, balance training, and modalities

  • Supervise and direct Physical Therapist Assistants (PTAs) and rehabilitation aides

  • Monitor resident progress and modify treatment plans as needed to achieve optimal outcomes

  • Complete accurate and timely documentation, including evaluations, progress notes, recertifications, and discharge summaries

  • Ensure compliance with Medicare (Part A & B), PDPM guidelines, and state/federal regulations

  • Participate in interdisciplinary team meetings, care planning, and discharge planning

  • Educate residents, families, and caregivers on treatment plans, safety, and home exercise programs

  • Maintain productivity standards while delivering quality patient care

  • Ensure a safe and effective therapy environment

Qualifications
  • Doctor of Physical Therapy (DPT) or Bachelor’s/Master’s degree in Physical Therapy from an accredited program

  • Current Physical Therapist license in the state of practice

  • Previous experience in a skilled nursing facility or geriatric setting preferred

  • Strong knowledge of rehabilitation techniques, equipment, and safety protocols

  • Familiarity with Medicare regulations, PDPM, and documentation requirements

  • Excellent communication, clinical reasoning, and interpersonal skills

  • Ability to supervise and mentor support staff

Work Environment
  • Skilled nursing / long-term care setting

  • Frequent standing, walking, bending, and assisting residents with mobility

  • May require lifting or supporting residents using proper body mechanics and equipment
