Responsibilities:

• Provide physical therapy services to skilled and part B residents, including evaluations, treatments, and education on a PRN basis

• Develop and implement individualized treatment plans to address patients' physical therapy needs and goals

• Participate in interdisciplinary team meetings to ensure effective patient care and communication

• Maintain accurate and up-to-date records of patient progress and treatment plans

• Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes

Requirements:

• Minimum of 0 years of experience as a Physical Therapist

• Current licensure as a Physical Therapist in the state of Oklahoma

• Other qualifications will be discussed during the interview process
