Berkshire Place is a fast-paced skilled nursing facility providing exceptional rehab services to both short-term and long-term residents. This is a great opportunity for a PT who values autonomy, collaboration, and a positive clinical culture.
Provide patient-centered, functional-focused care by:
• Completing evaluations and developing individualized plans of care
• Delivering skilled PT interventions aligned with physician orders and functional goals
• Assessing strength, mobility, balance, gait, endurance, and safety awareness
• Establishing realistic and measurable short- and long-term goals
• Completing evaluations, progress notes, recertifications, and discharge documentation accurately and timely
• Educating residents and families on treatment plans, mobility strategies, and safety
• Communicating changes in resident status to nursing, physicians, and the interdisciplinary team
• Collaborating closely with OT, SLP, nursing, and facility leadership
• Maintaining compliance with HIPAA, confidentiality, and resident rights
• Performing other duties as assigned by the Director of Rehabilitation
• Graduation from an accredited Physical Therapy program
• Active Physical Therapy license in the state of Rhode Island
• Skilled nursing and/or geriatric experience preferred, but not required
• Strong communication skills and a team-oriented mindset
