Join a growing Home Health team that is dedicated to helping patients regain strength, mobility, and independence in the comfort of their own homes. We're seeking a compassionate, motivated Physical Therapist (PT) who enjoys working one-on-one with patients and making a lasting impact every day.

Position Responsibilities:

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans
• Provide skilled physical therapy services in patients' homes
• Help patients improve strength, balance, mobility, endurance, and overall function
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
• Monitor patient progress and modify treatment plans as needed
• Complete accurate and timely documentation
• Collaborate with physicians, nurses, and the interdisciplinary Home Health team to provide exceptional patient care

Qualifications:

• Current Indiana Physical Therapist (PT) license
• Home Health experience preferred, but not required
• Strong clinical assessment and communication skills
• Compassionate, dependable, and patient-focused
• Valid driver's license and reliable transportation
