Aventura at Walton Hills is seeking a PRN Physical Therapist Assistant (PTA) to join our in-house rehabilitation team. This is an excellent opportunity for a PTA who enjoys working in a Skilled Nursing Facility/Long-Term Care setting and values flexibility while making a meaningful impact on residents' lives.

• Provide physical therapy treatments under the direction and supervision of a Physical Therapist.

• Assist residents in achieving their highest level of functional independence.

• Accurately document treatments and resident progress utilizing Net Health EMR.

• Collaborate with an interdisciplinary team to develop and implement individualized care plans.

• Maintain compliance with all federal, state, and facility regulations.

• Current Ohio PTA license in good standing.

• Skilled Nursing Facility (SNF) and Long-Term Care experience preferred.

• Experience with Net Health preferred.

• Strong communication, organization, and teamwork skills.

• Ability to work flexible PRN hours, including weekday, weekend, holiday, and vacation coverage as needed.
