Physical Therapist (New Graduate Welcome) – Skilled Nursing FacilityAbout the Role

We are seeking a compassionate and motivated Physical Therapist (PT) to join our rehabilitation team in a skilled nursing facility. New graduates are encouraged to apply! We are willing to train the right candidate and provide mentorship to help develop clinical skills and confidence in a post-acute care setting.

This is an excellent opportunity to work with an interdisciplinary team dedicated to helping residents regain mobility, improve function, and enhance their quality of life.

 

Responsibilities
  • Evaluate residents and develop individualized treatment plans.

  • Provide physical therapy services to residents recovering from illness, injury, surgery, or functional decline.

  • Establish realistic goals and monitor progress toward achieving optimal outcomes.

  • Document evaluations, treatment plans, progress notes, and discharge summaries in accordance with facility and regulatory requirements.

  • Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other healthcare professionals.

  • Educate residents, families, and caregivers on treatment plans and home exercise programs.

  • Participate in care planning meetings and interdisciplinary team discussions.

  • Ensure compliance with all federal, state, and facility policies and regulations

     

QualificationsRequired:
  • Graduate of an accredited Physical Therapy program.

  • Current or eligible Physical Therapist license in the state of employment.

  • Strong communication and interpersonal skills.

  • Ability to work effectively in a team-oriented environment.

  • Passion for helping older adults improve their mobility and independence.

Preferred:
  • Previous clinical rotations or experience in skilled nursing, rehabilitation, geriatrics, or long-term care.

  • Experience with electronic medical records (EMR).
