Evaluate and treat residents with cognitive and/or ADL physical functioning disorders
Develop and implement individualized plans of care using evidencebased practice
Provide skilled, medically necessary therapy to maximize recovery and functional independence
Administer rehabilitation interventions including therapeutic exercises, activities, ADL retraining, modalities, and assistive devices
Monitor resident progress and adjust treatment plans accordingly
Evaluate treatment effectiveness and recommend modifications or referrals as needed
Educate residents, families, and caregivers regarding treatment plans and outcomes
Communicate effectively with the interdisciplinary team regarding progress and barriers
Complete accurate, timely clinical documentation and treatment minutes
Provide thorough discharge summaries per regulatory and clinical standards
Participate in care conferences, rehab meetings, and required trainings
Supervise and support PTAs, rehab aides, and students as assigned
Maintain compliance with state practice acts, professional ethics, and facility policies
This position reports directly to the Director of Rehabilitation Services.
What We’re Looking for in YOUBachelor’s degree or higher in Physical Therapy or related field
Active state Physical Therapist license
2+ years of experience in a skilled nursing facility preferred
Strong clinical reasoning and documentation skills
Passion for working with seniors and improving functional outcomes
Ability to work collaboratively in a multidisciplinary team
