Bradford Hills Nursing & Rehabilitation Center is seeking a dedicated and compassionate Full-Time Physical Therapist (PT) to join our in-house rehabilitation team. This is an excellent opportunity to work in a collaborative Skilled Nursing and Long-Term Care environment focused on delivering exceptional patient outcomes.

Position Details

• Full-Time

• In-House Rehabilitation Program

• Skilled Nursing Facility (SNF) / Long-Term Care (LTC)

• Supportive interdisciplinary team

Qualifications

• Current Pennsylvania Physical Therapist license or eligibility for licensure

• Strong clinical, communication, and documentation skills

• Commitment to patient-centered care

• SNF/LTC experience preferred, but not required

Technology

• Net Health EMR

• PointClickCare (PCC)

Responsibilities

• Evaluate residents and develop individualized treatment plans

• Provide skilled physical therapy services to improve mobility, strength, balance, and functional independence

• Collaborate with physicians, nursing staff, and rehabilitation team members

• Complete timely and accurate documentation

• Ensure compliance with facility, state, and federal regulations
