Job Summary:

We are seeking a Registered Nurse (RN) to provide high-quality, patient-centered care to our residents. As a Registered Nurse (RN) at Clarion Nursing and Rehab, you will be responsible for assessing, planning, implementing, and evaluating care for our residents. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.

Responsibilities:

• Assess the physical and emotional needs of residents and develop individualized care plans

• Implement and evaluate care plans in collaboration with our interdisciplinary team

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring

• Collaborate with other healthcare professionals to coordinate resident care

• Maintain accurate and detailed records of resident care and treatment

• Participate in quality improvement initiatives to enhance resident care and services

• Provide education and support to residents, families, and caregivers

Requirements:

• Current licensure as a Registered Nurse (RN) in the state of Pennsylvania

• Strong communication and interpersonal skills

• Ability to work effectively as part of an interdisciplinary team

• Willingness to learn and adapt to new procedures and technologies

• Ability to provide high-quality, patient-centered care to residents with diverse needs and backgrounds
