Hospice RN Case Manager Responsibilities:

As our Rn Case Manager, you'll orchestrate meaningful care that touches hearts and creates lasting comfort for families. This role goes beyond medical tasks – you'll be the steady, compassionate presence that makes all the difference. Your responsibilities will include:

• Leading with heart: Guiding a multidisciplinary team in creating personalized care experiences

• Being the advocate: Championing each patient's unique needs, preferences, and wishes while supporting their loved ones

• Creating meaningful connections: Building relationships that matter, providing counseling and emotional support when it's needed most

• Making every day count: Developing and implementing care plans that prioritize quality of life and comfort

• Telling their story: Documenting each patient's journey with care, capturing progress and precious moments

Your Impact Zone:

As an RN Case Manager, you'll travel throughout Miami/Darke counties, bringing healing and hope directly to patients in their homes, assisted living facilities, long-term care facilities, and hospitals –wherever they feel most comfortable.

Hospice RN Case Manager Requirements:

• Active RN License

• Valid Driver's License

• 1+ years hospice experience

Preferred Qualifications:

• BLS Certification

• Hospice and Palliative Medicine background
