Job Description
Brookside Multicare Nursing Center is looking for an experienced MDS assessor or coordinator, who is also well versed in PDPM, to join our MDS team. Brookside Multicare is a large long/short term care facility, with an extremely diverse population, including dementia/Alzheimer's, behavior, and pediatric specialized units, as well as a subacute rehab unit. This is a great place to exercise your MDS muscles! We are located a few blocks from the Smithtown LIRR station, just a short distance from St. Catherine of Siena Medical Center.
Candidates must have a current NYS RN License, and at least 3 years' experience in MDS, with demonstrable understanding of PDPM and how it impacts the facility's reimbursement. Our MDS department works very closely with all members of the interdisciplinary team, so excellent communication skills are a must!
