Hospital Liaison

We are seeking a dynamic and results-driven Hospital Liaison to serve as the primary point of contact between our facility and local hospitals, physicians, and healthcare organizations. This role is key to fostering relationships, coordinating admissions, and ensuring high-quality patient transitions.

Key Responsibilities:

• Develop and maintain strong relationships with hospital discharge planners, case managers, and physicians

• Promote our facility’s services and specialties to referral sources

• Coordinate patient referrals and admissions in collaboration with clinical staff

• Provide timely communication regarding bed availability and patient placement

• Conduct on-site hospital visits and community outreach

• Ensure a smooth transition process for patients and their families

Qualifications:

• Experience in healthcare marketing, admissions, or case management (preferred)

• Knowledge of skilled nursing, rehab, or long-term care services

• Strong interpersonal and communication skills

• Ability to build and maintain professional relationships

• Self-motivated with excellent organizational skills
