As an MDS Coordinator RN, you will play a critical role in ensuring the seamless coordination of patient care by facilitating the Minimum Data Set (MDS) process. Your responsibilities will include:

• Collecting and reviewing patient information to complete the MDS assessment

• Ensuring accurate and timely completion of the MDS assessment

• Maintaining accurate and up-to-date records of patient care

• Collaborating with the interdisciplinary team to develop individualized care plans

• Providing education and support to patients and families

• Participating in quality improvement initiatives to optimize patient outcomes

Requirements:

• Current RN licensure in the State of Pennsylvania

• 1+ years of experience or related experience in long-term care or rehabilitation setting

• Ability to work effectively in a fast-paced environment

• Excellent communication and interpersonal skills

• Strong analytical and problem-solving skills
