What You’ll Do

• Perform initial and ongoing client assessments

• Develop and update individualized care plans

• Provide clinical oversight and guidance to care teams

• Communicate effectively with clients, families, and healthcare professionals

• Ensure compliance with state regulations and agency standards

• Complete routine medication set-ups for clients

Who You Are

• Licensed Registered Nurse (RN) in Colorado

• 2+ years of nursing experience (home health or hospice preferred)

• Current CPR certification

• Strong assessment and communication skills

• Valid driver’s license and reliable transportation

• Ability to work independently and manage multiple priorities
