Home Dialysis Training Nurse
Responsible for the day-to-day operation of the dialysis home training facility and for the oversight and function of
Licensed Practical Nurses and Patient Care Technicians providing the direct and indirect nursing care needs of an
assigned of home patients primarily of an adult/geriatric ESRD patient population. Serves as leader, role model
and mentor to direct care personnel providing support, instruction, and direction in the delivery of dialysis therapy.
This RN is ESRD Dialysis experienced in and able to perform home hemodialysis and peritoneal dialysis treatment
procedures in accordance with facility policy. Utilizes nursing process to assess individual patient needs,
determine, direct, and implement a plan of care, evaluate and revise plan according to changing patient care
needs. Initiates and provides on-going patient and family education and includes patient/family in care planning,
using appropriate forms to document care provided. Coordinates the efforts of members of the interdisciplinary
team to ensure continuity of care for each patient.

Job Summary
We are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.
Responsibilities
- Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
- Administer medications and monitor patients for side effects or reactions
- Document any changes in patient conditions and report on changes
- Maintain accurate records of patient care and discuss observations with the supervisor
Qualifications
- Currently licensed Registered Nurse (RN) in state of employment
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Valid driver’s license
- First aid/CPR certification required
Experience
One Year or more experience as a Registered Nurse, plus three months experience as a Registered Nurse in the
specific modality of hemodialysis (HD) and/or peritoneal dialysis (PD). If responsible for both the home
hemodialysis (HHD) and PD programs, the RN has at least 3 months experience in each respective modality.
