Home Health Physical Therapist- West Valley

The Home Health Physical Therapist provides expert physical therapy services to adult patients within their homes, managing an independent caseload and ensuring personalized care. This role requires frequent travel to patient residences and thorough documentation using electronic health records to track progress and coordinate with healthcare providers effectively.

Responsibilities

• Conduct patient assessments to evaluate physical condition and needs
• Develop and implement individualized treatment plans
• Perform home visits to deliver therapy and monitor patient progress
• Educate patients and caregivers on therapy exercises and health maintenance
• Coordinate care efforts with other healthcare providers
• Maintain accurate and timely documentation in electronic health records

Required Qualifications

• Licensed Physical Therapist (PT)
• Valid Physical Therapist (PT) license

Preferred Qualifications

• 1+ years of experience in physical therapy
• Strong skills in patient assessment and treatment planning
• Effective patient education techniques
• Excellent time management abilities
• Proficient in documentation and electronic health records
