PRN Physical Therapist

Provide skilled therapy intervention and treatment to patients in a home setting. Including, appropriate utilization of treatment plans and assessment skills. Monitor carefully the patient status and progress toward goals as well as compliance with physician’s orders. Responsible for educating patients and caregivers while preparing the patient/caregiver for discharge as appropriate.

Responsibilities

• Assess patient condition and functional abilities
• Develop and implement individualized treatment plans
• Administer therapeutic exercises and rehabilitation techniques
• Educate patients on care and recovery practices
• Document patient progress accurately using EHR systems
• Utilize therapeutic equipment safely and effectively
• Collaborate with multidisciplinary team members and supervise staff
• Ensure compliance with safety protocols and regulations
• Fulfill on-call duties and travel to patient residences as needed

Required Qualifications

• Minimum 1+ years' experience in physical therapy
• Bachelor's degree in physical therapy
• Licensed Physical Therapist (Ohio)
• Proficient in-patient assessment and rehabilitation techniques
• Strong communication and empathy skills
• Experience in team collaboration
• Accurate and thorough documentation capabilities
