RN Case Manager: Home Health & Hospice

Seeking compassionate, skilled Registered Nurses to join our Home Health and Hospice teams. In this role, you will provide hands-on clinical care, coordinate individualized care plans, and build trusted relationships with patients and families throughout Litchfield County and the Greater Farmington Valley.

What You’ll Do

·         Perform comprehensive nursing assessments.

·         Develop and manage individualized care plans.

·         Provide skilled nursing care in patients’ homes.

·         Educate patients and caregivers.

·         Collaborate with physicians and interdisciplinary team members.

·         Document patient care accurately and efficiently.

·         Respond to changing patient needs with clinical judgment and compassion.

Qualifications

• Active Connecticut RN license

• Current CPR certification

• Valid driver’s license and reliable transportation

• Strong communication and assessment skills

• Minimum of one year of home care and/or hospice experience preferred.
