MDS Coordinator

Description:

• The RN MDS Coordinator is responsible for assuring the timely completion of accurate MDS assessments that are in compliance with Federal and State guidelines.

• The RN MDS Coordinator is a team leader responsible for overseeing MDS assessors, making sure all aspects of the MDS are in compliance with state and Federal guidelines such as, scheduling care plan meetings, ensuring accurate diagnosis are placed in the EMR, certifications, Advance Beneficiary Notices, MDS Submissions, an attending morning huddle and Utilization Review meetings.

• The RN MDS Nurse is responsible to ensure accurate scores are obtained via the MDS to accurately reflect the residents and communication with the interdisciplinary team.

Requirements include:

• Registered Nurse with a current NYS Nursing License

• Must meet the requirements of the Department of Health

• Must be available Monday through Friday 9-5
