Responsibilities

• Coordinate and manage the Minimum Data Set (MDS) process for patients, ensuring timely and accurate completion of assessments, care plans, and revisions

• Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet patient needs and goals

• Review and apply knowledge of regulations, policies, and guidelines to ensure compliance with MDS requirements and other relevant standards

• Maintain accurate and detailed records of patient assessments, care plans, and progress

• Participate in quality improvement initiatives to enhance patient care and outcomes

• Provide education and support to patients, families, and caregivers to promote informed decision-making and positive outcomes
