Wellbound Certified Home Health Agency is seeking a Full-Time Home Care Educator to represent and educate our Case managers in the field.

THIS POSITION REQUIRES TRAVELING

Please note that you be required to travel locally.

Responsibilities:

• Work hand in hand with Case Managers in identifying problems and areas for improvement.

• Go to buildings to help identify problems with field clinicians.

• Educate and work with clinicians to correct their documentation.

• Review Patient charts.

• Review and complete orders.

Requirements:

• Valid NYS RN License.

• 5+ years of Home Care experience (Required)

• Knowledge in home care regulations- including Medicare and NYS regulations

• Case Management experience(Preferred)

• Clinical Experience(Preferred)
